Effective July 1, 2004 new prescription paper regulations went into effect for prescriptions written for
C-II medications.  Prescription pads must be printed on security paper by authorized printers.

The California Department of Justice is no longer issuing triplicate forms. Rx forms used for
prescribing Controlled Substances must be purchased from a California State Pharmacy Board
approved printer.
